Since your line and the nose of the worm would likely catch more weeds than a weed guard on a 90 degree hook, I do not see this as a critical element of its design.
The easiest hack attempt (IMO) would be to use tube jig mold and add 24ga wire keeper to on the underside of the hook (similar to what I posted on a poison tail years ago)... this would allow the tube jig head to stay inside of a worm and not ruin the mold if you wanted to make tube jigs without a keeper.

An option with no mold modification would be to cut a slit (or core a hole) into the bottom of your worm and and spin the jig when running the point through so the hook and eye penetrate on the opposite side as the slit/hole.
