Looks like the Stand Up HD Jig, Item No. 3297, or 3341. SUHD-5-A. Still being made.

I think that could be the old "Slide Along" Head. I have that one and can check the numbers tonight.

Marc, is correct. Quite the historian. That is the Slide Along Jig last produced in 1999. It does appear that is was modified for a larger updated hook.
The initial design was based off of a walking sinker. It did very well for a few years, but declined enough to have it discontinued.

Mine was circa '92. If you watch e-Bay you will see these once in a while. Good design. I haven't poured these lately. I used to paint the a crawfish color and tip them with a 3" brown twin twister tail and drag them along the bottom in Sturgeon Bay and get Smallies. They are VERY tough to snag on hard bottoms.

Maybe Do-it could make something similar using todays hooks with a slot for a brush guard or perhaps a wire weed guard.
